Ajax Options: add 'timeout' option and 'onTimeout' callback

For Common options, add a 'timeout' option to specify a value in milliseconds when a request timeouts.
For Common and Responder callbacks, add a 'onTimeout' callback.

I got this working for a project following the lead of this old patch I found:
